  Dear Sir/Madam:

You are kindly requested to submit quotations for the following item(s) in accordance with the specifications listed below. Firms invited by IntraHealth to submit offers for the goods described below are under no obligation to do so. Offerors shall not be reimbursed for any costs incurred in connection with the preparation and submissions of quotes.

IMCI Registers:

with the following specifications:

Cover page:size:A3(length:42cm,width:31cm) printed on a hard cover  in display board(carton paille) of 1500 gsm in color and inside papers of A3 printed on both sides in black and white on offset paper of 80gsm.

Inside paper:

Register of A3, printed both sides in black and white  as follows:

ü  0-7 days5 pages printed in black and white both sides on offset papers (pink color) of 80grams.  

ü  1 week to 2 months: 10 pages                

       On off set papers (yellow color) of 80gsm.

ü  2-59 Months : 345 pages printed in black and white both sides on offset paper of 80gsm.

ü  To put one paper separating 3 categories of ages.

Total pages of each book:  360 pages with one(1) additional  blank copy of 150grams after cover(at the beginning of the book pages).

Binding type: perfect binding

Ribbon(ruban): dark blue color(the color of the ribbon may change.

CONDITIONS REQUIREMENTS

 

Quotation should be accompanied by the following:

1. Cover letter, signed and stamped  by an authorized representative of the bidder.

2. Full address of bidder(printing house)  with photos of equipment which the bidder will use to execute the tender. IntraHealth/USAID Ingobyi Activity when deemed necessary, may visit the potential bidder to verify the equipment before award.

3. Company registration certificate

4.VAT registration certificate

5.Valid Tax clearance certificate (any bidder with tax arrears will not be considered)

6. Valid RSSB  clearance certificate

7. Proof of using EBM invoice.

8.Provision of at least three(3)references  for similar services(recommendations with corresponding  contracts) with a value of 10 million Rwandan francs and above for which the bidder has successfully performed in the last recent 5 years. At least one of the references should come from a reputable INGO.

Any bidder missing any of the above documents will be disqualified

Unless otherwise requested, quote on each item separately. Quotations should clearly state unit price and total price. Quotations must be typewritten or in ink and submitted on company letterhead.

IntraHealth reserves the right to a) reject any and all offers, in whole or in part, for any reason whatsoever, b) waive immaterial requirements, and c) pursue purchasing in a manner that is in the best interest of IntraHealth.

 United States law prohibits transactions with, and the provision of resources and support to, individuals and organizations associated with terrorism. The supplier must ensure compliance with these laws in any resultant contract from this RFQ.

SUBMISSION DATE AND TIME: February 5, 2021 before 5:00 PM Kigali local time.

IntraHealth will respond to any question received prior to the due date for the quotes, and may, at its sole discretion, respond to requests received later than the due date. The question(s) and response(s) will be sent to all vendors who have requested the RFQ documents. IntraHealth may extend the closing date to ensure offerors have adequate time to consider answers and reply accordingly.  Late quotes will not be accepted or considered.
